AdventHealth Zephyrhills in Florida is seeking a registered nurse focused on discharge planning and care coordination. You will lead multidisciplinary rounds, educate patients and families about care options, and coordinate post-acute services to ensure timely, smooth transitions.

The role requires RN licensure, 2+ years in hospital nursing, and experience with discharge planning and CMS standards; BLS and CCM/ACM certifications are preferred.

Qualifications

• RN with required licensure and discharge planning knowledge.
• Associates degree required; Bachelor's preferred.
• Experience in case management or utilization management preferred.

Responsibilities

• Participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness impact and resources.
• Assesses readmission causes and facilitates care conferences.
• Documents discharge plans and coordinates post-acute services.
• Ensures timely discharge with appropriate care coordination.
