Qualifications

• RN license required and current in Florida.
• Minimum 2+ medical/hospital nursing experience required.
• Proven ability to coordinate discharge planning with multidisciplinary teams.
• Experience with post-acute care and community resources is preferred.

Responsibilities

• Participate in multidisciplinary rounds to review patient status and discharge plans.
• Educate patients and families on care options, resources, and financial impacts.
• Assess readmission reasons and develop contingency discharge plans.
• Document discharge planning evaluations and any barriers to progression.
• Coordinate with payors for post-acute care authorization when needed.
• Ensure timely communication of discharge information for continuity of care.
