Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support care coordination for members across settings.

You will work with a multidisciplinary team to ensure quality, cost-effective care and progress toward desired outcomes, including home visits and community resource integration. The role requires 2+ years in health care, an active RN license, valid driver's license, and proficiency with EMR and HIPAA.

Qualifications

• At least 2 years experience in health care, preferably care management.
• RN license active and unrestricted in state of practice.
• Valid driver’s license, reliable transportation for travel requirements.
• Understanding of EMR and HIPAA compliance.
• Demonstrated knowledge of community resources.
• Ability to work proactively with attention to detail.
• Ability to work in diverse settings and with varied personalities.
• Strong verbal and written communication skills.
• Ability to work independently with minimal supervision.

Responsibilities

• Completes comprehensive assessments of members per timelines and identifies care management candidates.
• Develops and implements care coordination plans with member, family, physician and team.
• Performs telephonic, in-person or home visits as required.
• Monitors care plan effectiveness, documents interventions and adjusts goals.
• Manages an ongoing member caseload with regular outreach.
• Promotes integration of services across behavioral health and LTSS.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina guidance to support changes.
• Assesses barriers to care and provides coordination assistance.
