Job summary

Qualifications

• 2+ years in health care, care management or related setting.
• RN license active and unrestricted in state of practice.
• Valid driver’s license and reliable transportation for travel.
• Familiarity with EMR and HIPAA requirements.
• Strong communication and coordination skills.

Responsibilities

• Assess member needs and qualify for care management within timelines.
• Develop and implement care coordination plans with care team and member.
• Perform telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on progress and outcomes.
• Maintain ongoing member caseload and outreach.
• Coordinate services across behavioral health and LTSS as needed.
• Lead ICT meetings and collaborate with the care team.
• Provide resources and consult with peers as required.
