Job summary

Molina Healthcare, Inc. is seeking an RN Care Manager to support care management and care coordination across the continuum. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and contribute to a strategy for quality, cost-effective care.

Responsibilities include assessing members, developing care plans with stakeholders, and conducting visits. Travel up to 40% may be required; strong communication and organizational skills are essential.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of education and experience.
• Registered Nurse (RN) with active license in state of practice.
• Valid and unrestricted driver's license and reliable transportation for job-related travel.
• Understanding of EMR systems and HIPAA regulations.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision and maintain accuracy.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines eligibility for care management.
• Develops and implements care coordination plan with member, caregiver, physician and health professionals.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and tracks goal achievement.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and home resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guidelines to support change.
• Assesses barriers to care and provides coordination and assistance.
• May provide consultation and resources to peers as needed.
• Care manager RNs may handle complex cases and medication regimens.
