Job summary

Molina Healthcare, Inc. is seeking an RN for care management to coordinate member care across disciplines and settings. You will perform assessments, develop care plans, and support members through visits and ongoing monitoring.

The role requires independent work, strong communication, and the ability to travel 25–40% locally. Familiarity with EMR and HIPAA is essential for compliant care delivery.

Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of education and experience.
• Registered Nurse (RN) with active license in state of practice.
• Valid driver's license and reliable transportation for travel requirements.
• Understanding of EMR and HIPAA.

Responsibilities

• Completes comprehensive assessments of members per regulated timelines and determines who may qualify for care management.
• Develops and implements care coordination plan with member, caregiver, physician, and care team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and goal achievement.
• Promotes integration of services across behavioral health, LTSS, and community resources to enhance continuity of care.
• Facilitates ICT meetings and cross-team collaboration.
• Educates and motivates member using motivational interviewing and Molina guidelines.
• Addresses barriers to care and provides care coordination to members.
• May provide consultation and recommendations to peers as needed.
