Job summary

Molina Healthcare is actively seeking a Registered Nurse care manager in Florida to coordinate complex member care across disciplines. You will perform assessments, develop care plans, and travel as needed to deliver quality, cost-effective services.

Qualifications

• Minimum 2 years in health care, care management preferred.
• RN with active license.
• Valid driver’s license and auto insurance for travel.
• Knowledge of EMR and HIPAA.
• Ability to work independently with minimal supervision.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plan with member and team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document progress.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate ICT meetings and collaboration.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and provide assistance.
• May provide consultation to peers.
