Job summary

Somatus is seeking an RN Care Manager, VBC to manage high-needs CKD/ESKD patients. You will work with patients, nephrologists, and practices to develop and implement individualized care plans, remove barriers, and coordinate care across homes, clinics, and offices.

You will travel within your region, build trusted relationships with provider teams, and ensure seamless care transitions while delivering patient-centered outcomes.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years RN experience in a multi-disciplinary team and physicians.
• Valid BLS certification from a licensed facility.
• Renal or CKD/dialysis care experience as main focus of job.
• Reside in an area with reliable high-speed internet.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes and act as Somatus’ primary representative.
• Coordinate and lead regular interdisciplinary care team meetings with partnered practices.
• Establish and maintain positive relationships with patients and provider offices through in-person and telephonic engagement.
• Develop partnerships with provider teams to support clinical and operational goals.
• Educate provider practices on the Somatus program to align workflows.
• Deliver complete care coordination with in-person, telehealth, or phone communications.
