Job summary

Mercy Health is seeking an RN Care Manager at St Anne Hospital to coordinate care for patients and ensure safe, timely transitions across the continuum. The role emphasizes collaboration with the care team to address social determinants affecting wellbeing.

Required: at least 1 year in a clinical setting; BLS certification and RN license; BSN preferred for some Mercy Health sites; ambulatory/post-acute care coordination is valued.

Qualifications

• 1 year of clinical setting experience required.
• 3 years of acute care experience preferred.
• Ambulatory or post-acute care coordination experience preferred.
• Bachelor of Science in Nursing (BSN) is preferred/required for different Mercy Health sites.

Responsibilities

• Coordinate care for patients to support safe, seamless transitions across the continuum.
• Assess, plan, implement and evaluate options to meet health and related needs.
• Identify patients in need of active case management and promote right resources at the right time.
• Ensure documentation is complete, clear and timely in patient records.
