Job summary

Molina Healthcare, Inc. in Tampa, FL is seeking a Registered Nurse Care Manager to support care coordination across the continuum. You will collaborate with a multidisciplinary team to ensure quality, cost-effective member care and progress toward desired outcomes.

Responsibilities include comprehensive member assessments, developing and implementing care plans with the team, conducting telephonic, in-person or home visits, and monitoring plan effectiveness. Travel 25–40% may be required.

Qualifications

• 2+ years experience in health care, care management or related setting.
• Active Registered Nurse (RN) license.
• Valid driver's license and reliable transportation.
• Familiarity with EMR and HIPAA regulations.
• Strong communication and time-management skills.

Responsibilities

• Perform comprehensive member assessments within regulated timelines.
• Develop and implement care coordination plans with member, caregiver, physician and team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans, document interventions and adjust as needed.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary care team (ICT) meetings and collaboration.
• Support members using motivational interviewing and Molina guidelines.
• Address barriers to care and provide care coordination assistance.
• Care manager RNs may review complex member cases or medications.
