Qualifications

• At least 2 years of health care experience, preferably in care management or related settings.
• Active RN license in practice state.
• Understanding of EMR and HIPAA.
• Strong communication and organizational skills.

Responsibilities

• Complete comprehensive member assessments per timelines and determine eligibility for care management.
• Develop and implement care coordination plans with members, caregivers, physicians and teams.
• Conduct telephonic, in-person, or home visits as required.
• Monitor and adjust care plans based on outcomes and impacts.
• Maintain a regular member caseload and outreach for ongoing care.
• Promote integration of services across behavioral health, LTSS, and community resources.
• Facilitate interdisciplinary team meetings and collaboration.
• Provide education and support using motivational interviewing and clinical guidelines.
• Identify barriers to care and provide support to address concerns.
• May consult with peers and manage complex cases and medications as needed.
• Travel 25–40% may be required depending on state and contract requirements.
