Qualifications

• At least 2 years experience in health care, preferably in care management, or equivalent combination of relevant education and experience.
• Active RN license (unrestricted).
• Valid driver’s license with reliable transportation for travel requirements.
• Understanding of EMR systems and HIPAA compliance.
• Ability to work independently and proactively in diverse settings.
• Strong written and verbal communication skills and time-management.

Responsibilities

• Complete comprehensive member assessments within regulated timelines and determine eligibility for care management.
• Develop and implement care coordination plans with the member and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions and progress.
• Maintain ongoing member caseload and outreach.
• Promote integration of services across behavioral health, LTSS, and home resources.
• Facilitate ICT meetings and collaboration among teams.
• Educate and motivate members using motivational interviewing.
• Identify barriers to care and provide assistance or resources as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
• Travel may be required (25-40% depending on state/contractual requirements).
