Molina Healthcare is seeking a skilled Care Manager RN to support care coordination and interdisciplinary collaboration in Florida. The role emphasizes assessments, care planning, and proactive outreach to ensure patient outcomes while balancing costs.

Travel 25–40% may be required depending on state contracts and member needs, with focus on quality care and compliance. You'll work with the ICT, use EMR systems, and apply motivational interviewing to engage members and caregivers.

Qualifications

• RN with an active license in state of practice and minimum 2 years in health care or care management.
• Valid driver's license and reliable transportation for travel.
• Experience with EMR and HIPAA compliance.
• Ability to work independently, manage multiple projects, and adapt to diverse settings.
• Strong communication and interpersonal skills; proficient with Microsoft Office and online portals.

Responsabilités

• Completes comprehensive assessments within required timelines and determines eligibility for care management.
• Develops and implements care coordination plans with member, caregiver, physician, and team.
• Conducts telephonic, face-to-face or home visits as required.
• Monitors care plan effectiveness, documents interventions and goal achievement, and adjusts as needed.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS, and home/community resources.
• Facilitates ICT meetings and interdisciplinary collaboration.
• Uses motivational interviewing to educate and motivate change during member contacts.
• Assesses barriers to care and provides coordination assistance.
• May provide consultation to peers; some complex cases and medication reconciliation may occur.
• Travel 25–40% may be required based on state/contractual requirements.
