Qualifications

• Associate's Degree in Nursing or nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review OR completion of TIPS/Case Management Orientation upon hire.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Coordinate, negotiate and procure resources to meet patient and family needs.
• Communicate with medical staff and external resources to ensure appropriate utilization of resources.
