Job summary

St. Joseph Hospital Eureka is seeking an RN Care Manager for per diem, 8-hour variable shifts in Eureka, CA.

You will coordinate care for complex patients, communicate with the care team, and ensure alignment with clinical and financial standards to support optimal outcomes. The ideal candidate has an RN license in California, an ADN or nursing diploma, and 2 years of acute care experience, plus up to a year in care management or utilization review.

Qualifications

• ADN or nursing diploma required; California RN license upon hire.
• 2 years acute care experience in Medical-Surgical or inpatient setting.
• 1 year in care management or utilization review, or completion of related program upon hire.

Responsibilities

• Perform effective case management across the continuum of care for identified patients.
• Plan, coordinate, negotiate, and manage care and needs of complex patients and families.
• Communicate daily with medical staff and external resources to ensure appropriate resource use.
• Monitor medical records for variances and intervene to optimize outcomes.
