Job summary

Providence St. Joseph Hospital Eureka in Eureka, CA is seeking an RN Care Manager for a full-time day-shift (8-hour) role.

The position focuses on coordinating care for complex patients and families across the continuum of care, communicating with the medical team to ensure appropriate utilization of resources. The RN must have an Associate's Degree in Nursing or a nursing diploma and California RN license, with 2 years of acute care experience and 1 year in care management or utilization

Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California RN License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of care management or utilization review experience; or completion of TIPS or Case Management Orientation upon hire.
• Bachelor's Degree in Nursing or higher within 3 years of hire.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Coordinate, plan, negotiate, procure and manage care for complex patients and families.
• Communicate daily with medical staff, hospital personnel and outside resources for appropriate resource utilization.
• Monitor the medical record for variances to standards and intervene to achieve optimal outcomes.
