Job summary

Qualifications

• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review or successful completion of TIPS/Case Management Orientation upon hire.

Responsibilities

• Carry out effective case management functions for identified patients across the continuum of care.
• Coordinate, negotiate, procure and manage care for complex patients and families.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
