Job summary

Providence St. Joseph Hospital Eureka is seeking a Care Manager RN to oversee case management for complex patients, coordinating care across the continuum and collaborating with physicians and hospital staff.

The role requires California RN licensure, an Associate's degree or nursing diploma, and 2 years of acute care experience, plus 1 year in care management or a related program. It is a full-time, day-shift position with competitive benefits and growth opportunities.

Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review, or successful completion of TIPS program or Case Management Orientation upon hire.

Responsibilities

• Carry out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing care across the continuum.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources per clinical and financial standards.
• Monitor the medical record for variances to standards and intervene to achieve optimum outcomes for the patient and hospital.
• Support patient- and family-centered care and collaborate with care teams to optimize resource use and patient outcomes.

Education

Associate's Degree in Nursing or a nursing diploma

California Registered Nurse License

2 years acute care experience in Medical Surgical or Inpatient

1 year experience in care management or utilization review or TIPS/Case Management Orientation

Bachelor's Degree in Nursing within 3 years of hire
