Qualifications

Associate's Degree in Nursing or nursing diploma at hire.
California Registered Nurse License upon hire.
2 years of acute care experience in Medical Surgical or Inpatient setting.
1 year of experience in care management or utilization review in any setting, OR successful completion of TIPS program or Case Management Orientation Program upon hire.
Bachelor's Degree in Nursing or higher within 3 years of hire.

Responsibilities

Carry out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care.
Communicate on a daily basis with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources according to clinical and financial standards.
Monitor the medical record for variances to clinical and financial standards and intervene to achieve the optimum outcome for the patient and the hospital.
