Job summary

Providence St. Joseph Hospital Eureka is seeking a Care Manager RN to join our full-time Day shift team in Eureka, CA.

The role focuses on coordinating care for complex patients across the continuum, communicating with medical staff, and ensuring appropriate resource utilization per clinical standards. The ideal candidate will hold an Associate's Degree in Nursing, California RN license, and at least 2 years of acute care experience, plus 1 year in care management or related orientation

Qualifications

• Associates degree in nursing or nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review or completion of TIPS/Case Management Orientation upon hire.

Responsibilities

• Carry out effective case management functions for identified patients across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure appropriate resource utilization.
• Monitor medical records for variances to clinical/financial standards and intervene as needed.
