Providence St. Joseph Hospital Eureka in Eureka, CA, is seeking a Care Manager RN for a full-time day shift.

The role involves coordinating care for complex patients, communicating with medical staff and resources, and ensuring appropriate utilization of resources across the continuum of care. Requirements include an RN license in California, an associate degree or nursing diploma, 2 years acute care experience, and 1 year in care management or related program; preferred BSN and CCM/ACM

Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in Medical Surgical or Inpatient setting.
• 1 year of care management or utilization review experience or completion of TIPS/Case Management Orientation.

Responsibilities

• Carries out effective case management functions for identified patients across the continuum of care.
• Communicates daily with medical staff, hospital personnel and outside resources to ensure appropriate utilization of resources.
• Monitors medical records for variances to clinical and financial standards and intervenes to achieve optimum patient outcomes.
