Qualifications

• Associate's Degree in Nursing or a nursing diploma upon hire.
• California Registered Nurse License upon hire.
• 2 years of acute care experience in a Medical Surgical or Inpatient setting.
• 1 year of experience in care management or utilization review; or successful completion of TIPS or Case Management Orientation Program upon hire.

Responsibilities

• Carry out effective case management for identified patients across the continuum of care.
• Communicate daily with medical staff, hospital personnel and outside resources to ensure resource utilization.
• Monitor medical records for variances to clinical/financial standards and intervene as needed.
• Coordinate planning, negotiating, procuring care for complex patients and their families.
