Northwell Health is seeking a registered nurse to plan and deliver age-specific care using the nursing process across a defined patient group.

You will collaborate with physicians and health team members, lead patient care activities, and ensure timely assessments, interventions, and documentation.

This role emphasizes clinical judgment, safety, discharge planning, and education for patients and families, in a fast-paced hospital setting.

Competenze

• Graduate from an accredited School of Nursing.
• Bachelor’s Degree in Nursing preferred.
• Current license to practice as a Registered Professional Nurse in New York State.

Mansioni

• Assesses physiologic health status and reviews patient’s chart; documents care plan.
• Assesses psychosocial health status and patient/caregiver needs; identifies cultural requirements.
• Identifies patient outcomes and develops individualized goals.
• Forms and prioritizes plan of care; coordinates resources and documents care.
• Implements plan of care; ensures safety and timely interventions; reassesses as needed.
• Collaborates with physicians and care team; delegates appropriately; maintains communication.
