Job summary

Northwell Health is seeking a registered nurse to perform age-specific care using the nursing process across a designated patient group. The role requires collaboration with physicians and health-team members to coordinate procedures and treatments, directing care on the unit with clinical judgment and leadership.

You will assess physiologic and psychosocial health, identify outcomes, plan and implement care, and evaluate patient responses.

Qualifications

• Graduate from an accredited School of Nursing.
• Associate’s Degree in Nursing required.
• Bachelor’s Degree in Nursing preferred.
• Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut.
• BLS required; specialized certifications as needed.

Responsibilities

• Assesses patient’s physiologic health status.
• Assesses patient’s psychosocial health status.
• Identifies patient outcomes and develops measurement criteria.
• Forms the plan of care and sets priorities to achieve outcomes.
• Implements the plan of care and manages treatments and documentation.
• Evaluates care effectiveness and outcomes; supports performance improvement.
• Collaborates with care team and delegates tasks as appropriate.
• Performs Decubitus care and skin maintenance.
• Administers medications correctly and safely.
• Engages in patient and family education and discharge planning.
