Qualifications

• Conducts patient interviews and health status assessments across multiple systems.
• Develops and documents individualized care plans and goals.
• Collaborates with physicians and the care team to coordinate treatments.
• Maintains accurate, timely documentation and patient education.
• Assists with discharge planning and follow-up instructions.
