Northwell Health seeks an experienced Registered Nurse to lead age-appropriate care using the nursing process. The role emphasizes assessment, planning, implementation, and evaluation of patient care, with collaboration across the health team to ensure quality outcomes.

The RN will coordinate with physicians, monitor patient status, provide education to patients and families, and participate in discharge planning. NY State RN license is required; BSN preferred.

Formación

• Graduate from an accredited School of Nursing.
• RN license to practice in New York State.
• Bachelor’s Degree in Nursing preferred; enrolled in BSN within two years and RN degree within five years of entry date.

Responsabilidades

• Conducts patient interviews and explains policies and procedures to patients and families.
• Assesses gastrointestinal, cardiovascular, respiratory, renal and neurological health status and plans care accordingly.
• Documents plan of care and communicates health status to care team; coordinates with physicians and staff.
• Delivers and delegates care, maintains patient safety, and reassesses plans as needed.
• Provides discharge planning and patient education, including care instructions for home.
• Collaborates with physicians and other health professionals to implement treatment plans.
