Job summary

Wellstar Spalding Regional Hospital is seeking a Care Coordinator RN to assess transitional care needs, coordinate care across the continuum, and engage with patients and families to ensure discharge planning is efficient and appropriate.

This role collaborates with physicians, the care team, PAS and UM to address barriers, support care progression, and facilitate timely discharge while maintaining high-quality patient education and support.

Qualifications

• RN license in good standing.
• Minimum 1 year nursing experience in acute care.
• BLS certification required.

Responsibilities

• Assess clinical and psychosocial needs for discharge planning.
• Coordinate care progression and discharge planning with the care team.
• Collaborate with physicians, PAS, and UM to align on patient care and coverage.
• Document assessments, care plans, and post-acute referrals.
