Job summary

Wellstar North Fulton Hospital is seeking an RN Care Coordinator for a full‑time day shift to coordinate transitional care and ensure care needs are met across the continuum.

The CC RN will assess psychosocial needs, plan discharge, and collaborate with physicians and families to promote efficient resource use and timely discharge. This role emphasizes care progression, education, and interdepartmental coordination.

Qualifications

• Associate's Degree in Nursing from an accredited school of nursing with a Georgia RN License Required
• RN license (Georgia or Multi-State Compact) and BLS certification required upon hire
• Minimum 1 year nursing experience in the acute care setting

Responsibilities

• Assess psychosocial and functional status to plan transitional care.
• Coordinate discharge planning with physicians, patients, and family.
• Facilitate patient/family education and care conferences.
• Participate in interdisciplinary rounds and collaborate with the care team.
• Identify barriers to discharge and initiate referrals as needed.
• Document assessments and progress in the medical record.
