Job summary

The Wellstar Health System seeks a Care Coordinator RN (CC RN) to assess transitional care needs, coordinate across the continuum, and engage patients and families to ensure effective discharge planning. You will partner with the care team to optimize length of stay and resource use, guiding patients through progression of care and post-acute referrals.

Ideal candidates have at least 1 year of acute care nursing experience, strong communication, and the ability to collaborate with physicians,

Qualifications

• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication skill.
• Maturity, self-confidence, objectivity, and positive attitude.
• Self-directed with the ability to function well under stress in a fast-paced environment.
• Knowledge of local, state and federal regulations; ability to work with care teams and families.

Responsibilities

• Assessment and psychosocial risk factor evaluation; care progression and discharge planning.
• Discharges planning and coordination with physicians, care team, and payers.
• Participation in interdisciplinary rounds and post-acute referrals.
• Documentation of interactions and care progression in medical records.
• Identify barriers to timely disposition and communicate with leadership for resolution.
