Job summary

Avera Health is seeking an RN with strong leadership to oversee care delivery at Avera Prince of Peace-Skilled Nursing in Sioux Falls. The role focuses on coordinating MDS assessments, creating and updating care plans, and leading care conferences to uphold resident rights.

Ideal candidates have 1–3 years in long-term care and supervisory roles, with a commitment to Avera's mission and daily adherence to CARE principles, safety standards, and teamwork.

Qualifications

• Active RN license in the state of practice (Upon Hire).
• 1-3 years in long-term care and/or supervisory experience preferred.
• Commitment to care values and teamwork, confidentiality, safety compliance.

Responsibilities

• Coordinates, documents, and completes accurate MDS assessments and manages the MDS schedule.
• Completes initial admission care plans and updates care plans daily to reflect resident needs.
• Leads resident care conferences and advocates for resident rights.
• Provides nursing care per facility policies and maintains clinical competency.
• Participates in QAPI program and data collection with OSHA/DOH as applicable.
• Ensures CNA staff training per policy and state requirements.
• Daily rounding with staff and residents; schedules and supervises nursing team.
• Ensures successful compliance with annual DOH survey process.
• Supports organization mission, vision, values, and social principles.
