Job summary

Platte Health Center Avera is seeking a qualified Registered Nurse to serve as Patient Care Coordinator and assist the Director of Nursing with daily management at Platte Care Center. Responsibilities include supervising CNAs and RNs, coordinating MDS 3.0, and supporting admissions and resident care conferences.

The role requires SD RN licensure, LTC supervisory experience, and strong communication and organizational skills.

Qualifications

• Licensed RN in South Dakota.
• Experience in long-term care (LTC) supervisory preferred.
• Excellent communication and organizational skills.

Responsibilities

• Assist DON with daily departmental management.
• Supervise CNAs and nurses; coordinate staffing.
• Coordinate MDS 3.0 process and admissions.
• Attend and support resident care conferences.
• Ensure compliance with safety/risk management.
