Job summary

Responsibilities

• Educate and coordinate services during patient transitions from hospital to home care or hospice.
• Serve as home care expert to referral sources and agency staff.
• Obtain physician orders and complete required documentation for admissions.
• Maintain knowledge of relevant regulations and agency policies.
