Responsibilities

• Coordinates, documents, and completes MDS assessments.
• Manages the MDS schedule and ensures completion per guidelines.
• Completes initial admission care plan and updates daily.
• Collaborates with the interdisciplinary team and advocates for resident rights.
• Leads resident care conferences and ensures care standard compliance.
• Provides nursing care per facility policies and supervises CNA staff.
• Participates in QAPI and data collection from OSHA/DOH.
• Ensures CNA staff training per policy and regulatory requirements.
