Oak Street Health, part of CVS Health, is seeking a Registered Nurse to build strong patient relationships and coordinate care across the care team. You will assess, triage, educate, and support patients using standardized protocols to improve outcomes and align with patients’ values and needs.

You will report to the Practice Manager or Nurse Supervisor, participate in team care meetings, and contribute to a welcoming clinic environment.

Qualifikationen

• Active RN licensure in good standing in the applicable state.
• BLS certification.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information confidentially.
• US work authorization.
• Ability to assess patients without face-to-face interaction, strong communication and assessment skills.
• 2+ years of healthcare experience as an RN.

Aufgaben

• Provide competent nursing care following established protocols and safety standards.
• Respond to incoming calls and provide timely triage and guidance.
• Perform medication reviews and manage prescriptions as needed.
• Deliver patient education and direct care across in-person, phone, or group settings.
• Collaborate with the care team to implement and monitor care plans.
• Engage patients and caregivers to support preventive care and reduce ED visits.
