Job summary

Oak Street Health is seeking a Registered Nurse to build relationships with patients and coordinate care through standardized guidelines and pathways. You will ensure timely, competent nursing care via clinical assessment, education and interventions to improve patient outcomes.

The role emphasizes collaboration with the care team, delegation where appropriate, and delivering an exceptional patient experience.

Qualifications

• Active RN licensure in good standing with the applicable state.
• BLS Certification.
• Experience with Electronic Medical Records (EMR).
• Ability to maintain patient confidentiality and handle information confidentially.
• US work authorization.
• Strong ability to assess patients, including non-face-to-face interactions.

Responsibilities

• Provide competent nursing care in a safe and consistent manner.
• Respond to telephonic requests and exceed patient expectations.
• Provide triage and symptom management, in-person or remote.
• Utilize protocols for medication management, refills and prior authorizations.
• Review patient medications and update records as needed.
• Educate patients and deliver direct care, including chronic conditions.
