Qualifications

• Active, unrestricted RN licensure in the state you reside.
• BLS or CPR certification.
• Fluent written and spoken English.
• US work authorization.
• Electronic Medical Record (EMR) experience.
• Ability to maintain patient confidentiality and process information securely.
• Ability to assess patients without in-person contact when needed.

Responsibilities

• Support patient assessment and triage to ensure patients receive the right level of care and avoid unnecessary hospital visits.
• Perform skilled nursing procedures including wound care and vaccinations.
• Provide education on chronic conditions and preventative care for patients and caregivers.
• Review medications and renewals using standardized protocols and prior authorizations.
• Coordinate weekly transitions for hospital-to-clinic patient care.
• Collaborate with providers and the care team to manage complex care plans.
