Job summary

Oak Street Health in Sun City, AZ is seeking a Registered Nurse to build strong relationships with patients, coordinate care, and provide timely nursing care using standardized pathways and team collaboration to improve outcomes.

You will perform triage, medication reviews, education, and care-plan monitoring across in-person or telehealth encounters, reporting to the Practice Manager. 40 hours weekly, full-time, with comprehensive benefits. Eligible for CVS Health bonus and benefits.

Qualifications

• Active RN licensure in good standing in the applicable state.
• BLS Certification.
• EMR experience.
• US work authorization.
• Ability to assess patients without face-to-face interaction; strong communication and assessment skills.

Responsibilities

• Provide competent nursing care in a safe, consistent manner.
• Respond to telephonic requests and triage patient needs.
• Perform medication reviews, prescriptions refills, and prior authorizations.
• Provide patient education and direct care across in-person or group settings.
• Collaborate with care team and monitor care plan progress.
• Create a welcoming environment and delegate tasks to support care team.
