Job summary

Oak Street Health in High Point, NC is seeking a Registered Nurse to coordinate patient care and deliver seamless nursing services within our clinic. You will use standardized protocols, perform triage, education, and support care plans across multiple provider panels, while collaborating with physicians, nurses, and support staff to improve outcomes.

This full-time role requires NC RN licensure, CPR/BLS, EMR experience, strong communication, and a commitment to HIPAA and patient

Qualifications

• Active RN licensure in NC
• CPR/BLS certification
• EMR experience
• Clinic setting with hands-on patient care
• HIPAA compliance and patient confidentiality
• Strong communication and assessment skills
• Ability to work independently and multitask
• Team-oriented with interdisciplinary collaboration
• Lift up to 20 pounds and stand/walk most of shift
• US work authorization

Responsibilities

• Provide competent nursing care safely and consistently.
• Respond to telephonic requests and be responsive to patients' needs.
• Provide triage and symptom management to patients.
• Utilize standardized protocols for medication management and prior authorizations.
• Review patient medications and update as needed.
• Educate patients and deliver direct care for chronic conditions.
• Collaborate on progress of care plans across provider panels.
• Delegate tasks to team members to support patient needs.
• Build relationships with patients and caregivers for preventative care.
• Create a welcoming environment and participate in team huddles.
