Job summary

Oak Street Health is seeking a Registered Nurse to join our team in a value-based care setting. You will support patient assessments, perform protocol-driven chronic condition management, and collaborate with providers to reduce avoidable hospitalizations.

You will administer medications and vaccines, conduct point-of-care testing, and educate patients and families. The role emphasizes teamwork, patient safety, and timely clinical communications in a busy clinic.

Qualifications

• Active, unrestricted RN licensure in the state where you reside.
• BLS or CPR certification is required.
• US work authorization is required.

Responsibilities

• Support patient assessment and triage for walk-in and telephonic encounters.
• Manage chronic conditions and coordinate care with the care team.
• Perform wound care, medication administration, including vaccines.
• Educate patients and caregivers on chronic conditions and preventive care.
• Review medications and ensure patient safety and adherence.
• Conduct onsite testing (point-of-care labs, EKGs, specimen collection).
• Coordinate transitions for patients moving from hospital to clinic.
• Manage medication renewals and prior authorizations per protocol.
• Delegate to medical assistants and phlebotomists to optimize patient flow.
