Job summary

The role emphasizes adherence to clinical protocols and patient education, including chronic condition management. Responsibilities include triage, medication management, care plan implementation, and care-team collaboration.

Qualifications

• Active RN licensure in good standing in Pennsylvania.
• Current CPR and/or BLS certification.
• EMR experience.
• Clinic setting experience with vital signs, injections and vaccinations.
• Maintain patient confidentiality and HIPAA compliance.
• Strong communication and assessment skills.
• Ability to work independently and prioritize multiple tasks.
• US work authorization.

Responsibilities

• Provide competent nursing care in a safe and consistent manner.
• Respond to incoming telephonic requests and be responsive to patients.
• Provide triage and symptom management to patients regardless of location.
• Use standardized protocols for medication management and prior authorizations.
• Review patient medications and update as needed.
• Educate patients and provide direct care around chronic conditions.
• Collaborate with care team to monitor and progress care plans.
• Build relationships with patients and caregivers to support preventative care.
• Create welcoming environment focused on equity of care.
• Participate in care team meetings and daily huddles.
• Deliver exceptional patient experience through service and respectful care.
• Perform point of care testing and phlebotomy as needed.
