• Must maintain continuing education/licensing.
• Must remain in good standing with the appropriate licensing body.

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment protocols.
• Attend morning meetings/stand ups to facilitate team communications.
• Answer residents’ questions about care, illness progression, and treatment plans.
• Organize and prioritize daily work based on residents’ needs in their area.
• Complete documentation and perform utilization review activities.
• Coordinate care with residents, providers, facilities, and third‑party payers.
• Oversee admissions and discharge activities and coordinate referrals.
• Demonstrate professional, respectful interactions with residents, families, staff and partners.
