Qualifications

• Bachelor’s Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs/benefits.
• PCC knowledge.
• CCM certification a plus; continuing education/licensing maintained.

Responsibilities

• Communicate with residents and families about Plans of Care, PT/OT and treatment protocols.
• Attend morning meetings to coordinate care with the team.
• Answer residents’ questions about care, illness progression and plans.
• Monitor and adjust resident statuses based on changing needs.
• Prioritize daily work by assessing residents’ needs (admission, current, discharge).
• Complete required documentation and charting.
• Perform utilization review for appropriate, timely care.
• Coordinate care with residents, care providers, facilities’ financial services and third-party payers.
• Oversee admissions and discharge activities and coordinate referrals.
• Relate well with residents, families, staff and professional colleagues; promote cooperation.
• Manage time effectively in a high-volume, dynamic environment.

Education

Bachelor’s Degree in Nursing or Social Work

RN license preferred

LVN/LPN license acceptable

Two years clinical nursing experience preferred

PCC Knowledge

Medicare/Medicaid knowledge

CCM certification a plus
