Job summary

PACS is seeking a qualified Clinical Care Coordinator to coordinate care across residents, families, and care teams in a post-acute facility. You will participate in rounds, clarify plans of care, and ensure timely documentation.

The role requires a Bachelor’s degree in Nursing or Social Work, RN license preferred, LVN/LPN acceptable, and 2 years clinical nursing experience. Knowledge of Medicare/Medicaid is beneficial.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required or preferred.
• RN license preferred; LVN/LPN acceptable alongside licensure.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.
• Ability to coordinate care and document accurately.

Responsibilities

• Communicate with residents and families about plans of care and treatment.
• Attend rounds/stand-ups to facilitate team communication.
• Oversee admissions and discharge activities and coordinate referrals.
• Complete required documentation and utilization reviews.
• Work independently with sound judgment in interactions with providers.

Education

Bachelor’s Degree in Nursing or Social Work

Registered Nurse (RN) license preferred

Licensed LVN/LPN acceptable

Two years clinical nursing experience preferred
