Job summary

Winterhaven Kennels is seeking a qualified healthcare professional to coordinate resident care and support families in Florida. The role involves communicating plans of care, participating in daily team huddles, and ensuring accurate documentation.

The position requires a Nursing or Social Work degree, an RN/LPN/LVN license (RN preferred) and prior clinical nursing experience. Knowledge of Medicare/Medicaid is a plus, with PCC familiarity beneficial.

Qualifications

• Bachelor's Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• PCC Knowledge.

Responsibilities

• Communicate regularly with residents and families about plans of care and treatment protocols.
• Attend morning meetings to facilitate team communication.
• Monitor residents' statuses and adjust as needed.
• Complete required documentation and charting.
• Coordinate admissions and discharges.
• Oversee referrals to and from the facility.
• Maintain timely communication with physicians, payers and care teams.
• Ensure high-quality, cost-effective care.
