Job summary

PACS is seeking a dedicated healthcare professional to manage care coordination in a post-acute setting. The role involves communicating plans of care with residents and families, attending team stand-ups, and monitoring resident status to ensure safe, cost-effective care.

Responsibilities include organizing daily workflow, completing documentation, and coordinating with physicians, payers, and ancillary staff.

Qualifications

  • Bachelor's degree in Nursing or Social Work.
  • RN license preferred; LVN/LPN acceptable.
  • Two years of clinical nursing experience preferred.
  • Knowledge of Medicare/Medicaid programs and benefits.
  • Must maintain continuing education/licensing.

Responsibilities

  • Communicate with residents and families about Plans of Care, PT, OT and other treatment protocols.
  • Attend morning meetings/stand up to facilitate team communications.
  • Monitor and adjust resident statuses based on changing needs.
  • Organize and prioritize daily work by assessing new, current and discharging residents' needs in areas of responsibility.
  • Complete documentation as required; charting in timely manner.
  • Perform utilization review to provide appropriate, timely, and cost-effective care.
  • Coordinate care with residents, providers, facilities, financial services, and third-party payers.
  • Oversee admissions and discharge activities; coordinate referrals to/from the facility.
  • Maintain professional, respectful, honest interactions with residents, families, staff and partners.
