Job summary

PACS is seeking a qualified healthcare professional to deliver high-quality post-acute care coordination. The role emphasizes collaboration with residents, families, and a multidisciplinary team to create and implement care plans while controlling costs.

Strong communication and documentation skills are essential. The position requires nursing or social work education, licensure, and experience in a fast-paced setting.

Qualifications

• Education and/or Experience: Bachelor’s Degree in Nursing or Social Work, RN licensed or LVN/LPN acceptable.
• RN license preferred; CCM certification a plus; continuing education/licensing required.
• Two years of clinical nursing experience preferred; knowledge of Medicare, Medi-Cal and Medicaid programs helpful.
• PCC knowledge preferred; strong documentation and regulatory understanding.
• Must maintain licensure and remain in good standing with health department requirements.

Responsibilities

• Communicate with residents and families about Plans of Care, PT/OT and treatment protocols.
• Attend morning meetings to facilitate team communications.
• Answer questions about care, plans, illness progression and safety.
• Monitor and adjust resident statuses based on needs.
• Organize daily work and prioritize tasks in a high-volume environment.
• Complete documentation accurately and timely; perform utilization reviews.
• Coordinate care with residents, providers, facilities, payers and referrals.
• Oversee admissions and discharge activities and ensure smooth transitions.
• Maintain professional interactions with residents, families, staff and partners.
• Ability to work independently with sound judgment and effective communication.
