Job summary

RN preferred; LVN/LPN acceptable; strong time management and Medicare/Medicaid knowledge are important.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid, and related programs.

Responsibilities

• Communicate with residents and families about plans of care and treatment.
• Attend morning meetings to coordinate care with the team.
• Monitor and adjust residents' statuses based on changing needs.
• Complete documentation accurately and timely.
• Coordinate admissions, discharges, and referrals.
• Maintain professional interactions with residents, families, staff and providers.
