PACS is revolutionizing the delivery and leadership of post-acute healthcare and support services nationwide. This Case Manager position supports residents and families by coordinating Plans of Care, treatment plans, and utilization review to ensure timely and cost-effective care.

The role requires a Bachelor’s in Nursing or Social Work, RN/LVN licensure (RN preferred), and two years of clinical nursing experience; knowledge of Medicare/Medicaid is preferred.

Qualifications

• Bachelor’s Degree in Nursing or Social Work required.
• RN or LVN/LPN license preferred; must be in good standing.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medicaid programs and benefits.

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment plans.
• Attend morning meetings to facilitate team communication.
• Monitor resident statuses and adjust plans based on changing needs.
• Complete documentation and utilize review activities to ensure appropriate, timely care.
• Coordinate admissions, discharges, and referrals with all stakeholders.
