Job summary

Wellstar North Fulton Hospital is hiring an RN Care Coordinator for a full-time day shift. This role assesses transitional care needs, coordinates across the continuum and engages with patients and families to ensure successful discharge planning.

You will collaborate with the care team, utilization review, and physicians to manage care progression, document thoroughly in the medical record, and support efficient resource use in a fast-paced acute care setting.

Qualifications

• Associate's Degree in Nursing with GA RN license.
• Minimum 1 year nursing experience in acute care.
• Current BLS certification required.
• RN license via Multi-state Compact (Nurse Licensure)

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Collaborate with utilization review nurse, physicians and care team to plan discharge.
• Manage discharge planning and identify barriers to timely transition.
• Document in the medical record and track avoidable days for continuous improvement.
