Job summary

Qualifications

• Associate's degree in nursing from an accredited program is required
• GA RN license required
• Minimum 1 year of acute care nursing experience
• Excellent written and verbal communication; ability to work under stress and in a fast-paced environment
• Knowledge of local, state and federal regulations; knowledge of community resources; ability to collaborate with physicians and care teams

Responsibilities

• Assess chronic disease management needs and psychosocial risk factors for discharge planning
• Coordinate discharge planning with the care team and payers to ensure timely progress
• Facilitate communication among patient, family, physicians, and care team to support care progression
• Document assessments and care plans in the medical record; track avoidable days and trends
• Perform professional development and mentorship activities as required
• Perform other duties as assigned and comply with policies
